How do you create a great form for your employees to use in onboarding?
Label fields clearly and directly above the field
Add clear field names directly above each field or immediately to the left of the box. Although directly above the field is best, whichever you choose, be consistent on all forms. Changing could confuse an employee and create an issue with the onboarding process if they do not know how to proceed.
Maintain a single-column
Asking for information vertically and in a single column will help employees using smartphones and smaller screens. It also eliminates guesswork on tab order through your form.
Group Related Fields
Create logical groups so the user can interpret what is being asked in the form faster and manage requests in batches. Visual separation, like a line, should be considered when changing the type of information.
